Full Job Description
Job Summary

Advanced Practice Provider is responsible for obtaining complete and comprehensive patient histories, performing physical examinations, writing orders, responding to patient questions, and providing information to patients and their families. Exercises judgment within their areas of competence provided that a member of the supervising physician team shall have the ultimate responsibility for patient care.

Job Responsibilities
  • Order, perform and interpret laboratory tests, diagnostic and invasive procedures as appropriate.
  • Following physician observation and approval of technique, perform therapeutic or corrective measures as well as some outpatient family medicine procedures as needed.
  • Refer patients to appropriate specialists, services, and community resources (i.e. physical therapy, other subspecialists and social services).
  • Provide preventative care and counseling on health and wellness related conditions.
  • Prescribe appropriate medications as needed.
  • The supervising physician or physician designee reviews all patient encounters no later than (72) seventy-two hours after the APP has seen the patient. (100% of charts in first 3 years of employment; 50% of charts thereafter. IN code 25-27.5-6-1)

Education Requirements
  • Minimum: Graduate of an approved/accredited program
  • Preferred: Masters degree.

Experience Requirements
  • Minimum: One (1) year of clinical experience
  • Preferred: Three (3) years of experience in medical office

License/Certification Requirements
  • Valid license to practice in the state of Indiana
  • Controlled Substances Registration license in the state of Indiana
  • Basic Life Support (American Heart Association)
