Responsibilities

  • Assist patients with rehabilitation exercises as instructed by the Physical Therapist
  • Document patient progress and treatment notes according to regulatory guidelines
  • Maintain cleanliness and organization of the therapy area
  • Engage in interprofessional meetings related to patient care
  • Train nursing staff and Restorative Aides on therapy techniques
  • Communicate effectively about patient status with healthcare team
  • Provide education to patients' families on post-therapy care

We are looking for a Physical Therapist Assistant/PTA at Sunnyvale Post Acute! Join our dynamic rehab team!
  • Full time, Part Time and PRN Opportunities

Job Description:

Treat patients as directed by Physical Therapist.

Record daily treatment notes and weekly progress notes per PT Board.

Assist in maintaining department.

Participate in Patient Care and Rehabilitation Conferences, as needed.

Assist with cleaning and maintenance of treatment area.

Treat patients per the physician treatment plan.

Assist nursing department with training of Restorative Aides.

Communicate with supervisor and other health team members regarding patient progress, problem and plans.

Participate in in-services training program for other staff in the facility. • Record treatment changes per policy and procedures.

Instruct patient's families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.

Report any problems with department equipment so that it is maintained in good working order.

Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.

Required license or certification:
