Job Title: Physical Therapist (PT)Job Summary:The Physical Therapist provides comprehensive evaluation and treatment services to improve patients' mobility, strength, function, and overall quality of life. This role focuses on delivering patient-centered, holistic rehabilitation care, with an emphasis on treating adult and geriatric populations with complex medical and functional needs. The PT collaborates with an interdisciplinary care team across clinical and community-based settings.
Key Responsibilities:- Perform physical therapy evaluations and develop individualized treatment plans
- Implement therapeutic interventions to improve mobility, strength, balance, and functional independence
- Monitor and document patient progress and adjust care plans as needed
- Collaborate with physicians, nurses, and rehabilitation staff to coordinate patient care
- Educate patients and caregivers on exercises, mobility techniques, and fall prevention strategies
- Provide care across outpatient, acute, inpatient, or skilled nursing settings as assigned
- Participate in interdisciplinary care planning for complex patient populations
- Ensure compliance with clinical standards, safety protocols, and regulatory requirements
Required Qualifications:- Bachelor's degree in Physical Therapy (Master's or Doctorate preferred)
- Valid California Physical Therapist License or proof of license eligibility
- Minimum of two (2) years of clinical experience preferred, particularly with frail or elderly populations
- CPR and First Aid certification within 180 days of hire (preferred upon hire)
Preferred Experience:- Outpatient rehabilitation experience
- Acute care experience
- Inpatient rehabilitation experience
- Skilled nursing facility (SNF) experience
- Strong experience working with geriatric patients
Required Certification:- California Physical Therapist License
Ideal Candidate Profile:- Strong geriatric rehabilitation experience
- Experience in outpatient or acute care settings

- Pediatrics-only experience will not be considered
