About the Role

  • Conduct physical therapy evaluations and develop individualized treatment plans.
  • Provide person-centered physical therapy services that promote mobility, independence, and quality of life.
  • Develop, implement, and monitor Physical Management Plans (PMPs) and modify interventions based on individual progress.
  • Collaborate with interdisciplinary teams to develop and support individualized service plans.
  • Ensure services comply with CMS, federal, state, and agency regulations and professional standards.
  • Maintain timely and accurate clinical documentation, assessments, and Medicaid-related records.
  • Provide clinical supervision and guidance to Licensed Physical Therapist Assistants (LPTAs).
  • Train and educate direct support professionals and other staff on therapy programs and safe implementation of treatment plans.
  • Participate in quality improvement initiatives, staff development, and continuing education to support high-quality rehabilitation services.
  • This position is considered essential personnel and would be required to work in the event of an emergency or facility need.
  • All new hires are required to successfully complete the facility's two-week pre-service training program, held Monday through Friday from 8:00 a.m. to 4:30 p.m.


Skills and experience required:

We seek a candidate with experience working in a direct care setting with individuals with mental illness, developmental disabilities, and/or substance use disorders. We also seek the following:

  • Excellent verbal and written communication skills.
  • Ability to develop, implement, and evaluate comprehensive physical therapy treatment plans.
  • Ability to assess physical and psychosocial needs and implement appropriate therapeutic interventions.
  • Knowledge of evidence-based physical therapy principles and practices.
  • Ability to effectively serve individuals with behavioral health needs.
  • Extensive knowledge of rehabilitation services for individuals with intellectual and developmental disabilities.
  • Ability to follow written and verbal policies, procedures, and clinical guidelines.
  • Ability to analyze data and prepare concise clinical documentation and progress reports.
  • Ability to effectively present assessment findings and recommendations to interdisciplinary teams, families, and authorized representatives.
  • Proficiency with Microsoft Office applications (Word, Excel, Outlook, Teams) and Electronic Health Record (EHR) systems.
  • Graduation from a Physical Therapy program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE), or an equivalent program recognized by the Virginia Board of Physical Therapy.
  • Current licensure or eligibility for licensure as a Physical Therapist by the Virginia Board of Physical Therapy.

Bonus points if you have:

  • Experience providing physical therapy services to individuals with intellectual and developmental disabilities (IDD), behavioral health needs, or complex medical conditions.
  • Knowledge of supervisory principles and practices.
  • Familiarity with Applied Behavior Analysis (ABA) principles and their application within developmental disability services.
  • Demonstrated ability to train, coach, and mentor staff.
  • Experience coordinating workloads and assigning staff responsibilities.
