Qualifications

• Current Massachusetts Physical Therapist (PT) license required.
• Minimum of one (1) year of clinical experience.
• Afternoon availability is necessary.
• Preferred: Previous home care experience.
• Preferred: Availability Monday-Friday.

Responsibilities

• Evaluate patients' mobility, strength, balance, and functional abilities.
• Develop and implement personalized treatment plans for improvement.
• Provide therapeutic exercises, gait training, and fall prevention.
• Assess home environments and recommend safety modifications.
• Educate patients and caregivers on treatment and injury prevention.
• Collaborate with physicians and interdisciplinary teams.
• Complete accurate electronic documentation promptly.
