We are seeking a Full time, Part time or PRN SNF Physical Therapist to join our team at Bear Creek Senior Living in Colorado Springs, CO.
As a SNF Physical Therapist, you will play an important role in delivering the exceptional care and service that defines life at Stellar Senior Living.

Your RoleAs a Physical Therapist at Bear Creek Senior Living, you'll be a vital part of a compassionate team dedicated to enriching the lives of our residents. Your duties will include:
- Providing direct patient care, evaluating functional needs and outcomes, and consulting with other specialists
- Reviewing patients' functional needs and adjusting care plans to ensure optimal patient outcomes
- Designing and implementing individualized plans of care based on thorough assessments of potential development, incorporating participation by significant others whenever appropriate
- Conducting therapy practice within defined standards of care
- Directing the assistance of support staff to achieve patients' functional goals
- Communicating effectively with other professional and support staff to achieve positive patient outcomes
- Practicing facility and departmental policies and procedures, including quality improvement, safety, environmental, and infection control standards
- Documenting evaluations, treatment goals, and plans; regularly updating documentation and maintaining prescriptions and signatures
- Enhancing professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops
Qualifications- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current Physical Therapist license from the state of Colorado Board of Physical Therapy Examiners
- Knowledge and skills to care for the physical and developmental needs of the population age groups that will be receiving services
- At least one year of experience in a skilled nursing or rehabilitation setting preferred
- Ability to remain calm in emergency or crisis situations
- Strong interpersonal and communication skills
