Key Responsibilities: - Evaluate residents and develop individualized care plans
- Implement physical therapy treatment programs to restore function and reduce pain
- Work collaboratively with nursing and rehab staff to support holistic care
- Maintain accurate documentation in accordance with regulatory standards
- Educate residents, families, and caregivers on therapy exercises and goals
- Monitor and report progress to physicians and care team members
Requirements - Active New York Physical Therapist license (required)
- Graduate of an accredited Physical Therapy program
- Experience in a skilled nursing or long-term care setting preferred
- Strong interpersonal, communication, and organizational skills
- Empathy and passion for working with geriatric populations
