Physical Therapist

San Jacinto Valley Post Acute

The Physical Therapist is responsible for evaluating, developing, and implementing individualized treatment plans to improve mobility, reduce pain, restore function, and enhance quality of life. This role collaborates closely with an interdisciplinary team to deliver outstanding patient-centered care.
Essential Responsibilities
  • Evaluate residents and develop individualized treatment plans based on clinical findings and goals.
  • Provide therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence.
  • Monitor patient progress and modify treatment plans as appropriate.
  • Collaborate with physicians, nursing staff, therapy team members, residents, and families to ensure coordinated care.
