LTC Therapy is seeking a Physical Therapist (PT) to join our rehabilitation team. This position offers the opportunity to work with residents in a skilled nursing and post-acute care setting, helping them improve mobility, strength, balance, and overall functional independence.
Position Responsibilities:- Complete physical therapy evaluations and develop individualized treatment plans
- Provide treatment focused on mobility, gait training, transfers, strengthening, balance, endurance, and pain management
- Collaborate with PTA staff, nursing professionals, physicians, and interdisciplinary team members
- Monitor resident progress and adjust treatment plans as appropriate
- Educate residents and families on exercises, mobility techniques, and safety recommendations
- Maintain accurate and timely clinical documentation

Qualifications:- Active Physical Therapist license required
- Strong communication and clinical reasoning skills
