This role primarily serves patients in Hollister and surrounding communities throughout San Benito County--allowing you to stay close to home.
What You'll Do- Conduct comprehensive initial evaluations and assessments and promptly report findings to the physician/provider and Clinical Manager
- Develop, implement, and update individualized plans of care based on patient needs and physician orders
- Perform skilled physical therapy interventions, procedures, measurements, evaluations, and diagnostic tests in the home setting
- Complete initial evaluations, treatment documentation, and re-evaluations every thirty (30) days; update plans of care as needed
- Complete comprehensive assessments and OASIS documentation accurately and on a timely basis, as applicable
- Observe, document, and report patient response, progress, and any significant changes to the physician/provider and care team
- Communicate and collaborate with physicians, providers, and interdisciplinary team members to ensure coordinated, high-quality care
- Provide education and support to patients and their families to promote safety, independence, and long-term success
- Assist with orientation, supervision, and consultation for Physical Therapy Assistants, as assigned

Qualifications- Master's degree in Physical Therapy
- Active Physical Therapy license in good standing in the state of California
- At least one year of experience working as a Physical Therapist
- Must have a vehicle, a valid current driver's license, and automobile insurance
- Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively, and submit in a timely manner
- Home Health and Home Care Home Base documentation experience
