Job Description:

Locations
  • North: The Villages, Lady Lake, Belleview, Summerfield, Marion Oaks, Wildwood, Bushnell, Coleman, and Oxford
  • South: Clermont, Minneola, Groveland , Mascotte, Leesburg
  • All of Lake County

Schedule
  • Full Time: Monday - Friday, 8am - 5pm, flexible, rotating weekends on-call
  • NOTE: There may be part-time or PRN opportunities available
  • Develops patient-specific care plans with realistic, measurable goals in consultation with patients, families, and physicians that Considers the physical, cultural, psychosocial, spiritual, age-specific, and educational needs of the patient.
  • Reassesses patients at appropriate intervals, documents progress, and determines the effectiveness of therapy.
  • Observes and records patient activities and findings, reporting any changes or deviations in the plan of care to the physician. Instructs patients and caregivers on the use of assistive devices and establishes home programs.
  • Documents treatments accurately and timely in medical records and updates care plans as necessary. Interacts and participates in the education, quality improvement activities, role development, and orientation of agency personnel, promoting and supporting growth of others through precepting and mentoring.
  • Coordinates and directs the care of home patients requiring physical therapy, ensuring comprehensive assessment, planning, implementation, and evaluation.
  • Prioritizes and adapts home care caseloads to meet the changing needs of patients and families.
  • Performs evaluations to assess patients' functional abilities, and documents results in clinical records.
  • Selects, applies and modifies skilled interventions to achieve the best outcomes for patients, reassessing as needed.
  • Other duties as assigned.

Knowledge, Skills, and Abilities:
  • Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing [Required]
  • Home Care Regulations and Third-Party Reimbursement as it impacts care delivery [Required]
  • Demonstrated knowledge of physical therapy theory, modalities, treatments, and skills [Required]
  • Basic computer skills and knowledge of Microsoft Office. [Required]
  • Must be able to read, write, and speak conversational English. [Required]

Education:
  • Doctorate [Preferred]

Field of Study:
  • Physical Therapy

Work Experience:
  • 1+ year relevant clinical physical therapy experience [Required]
  • Recent, relevant experience in a Medicare-certified home health agency as a care-manager [Preferred]

Pay Range:

$34.71 - $64.55
