As a Community-Based Physical Therapist with Home Health Care, Inc., you'll treat patients where your expertise has the greatest impact-right in their homes. Every treatment session is personalized, every goal is functional, and every success is something your patients experience in their everyday lives.
What You'll Do- Evaluate patients in their homes and develop individualized treatment plans.
- Improve mobility, strength, balance, endurance, and overall functional independence.
- Educate patients and caregivers on exercises, adaptive strategies, and injury prevention.
- Collaborate closely with nurses, physicians, occupational therapists, speech therapists, and the interdisciplinary care team.
- Monitor patient progress, modify treatment plans, and document outcomes accurately.
- Empower patients to remain safe, independent, and confident at home.
What We're Looking For- Active Minnesota Physical Therapist (PT) license.
- Strong clinical reasoning and excellent communication skills.
- Passion for helping patients maximize independence in their own homes.
- Ability to work independently while managing a field-based schedule.
- Valid driver's license, reliable transportation, and willingness to travel throughout the Twin Cities Metro.
