Description:
We are looking for a Physical Therapist (PT) to support the treatment of illness and injury through the use of physical exercises and therapies.
This role assists in the rehabilitation and restoration of body functions to promote maximum client wellness and independence.
You will provide therapy programs for patients of all ages with developmental deficits, physical injury, or illness.
The position may include other responsibilities and duties based on the facility's needs and requirements.
Responsibilities include:
Coordinate PT services and provide assigned clients with direct therapeutic care.
Teach rehabilitation skills to clients and caregivers that promote maximum independent functionality and safety.
Provide preventive and rehabilitative PT procedures individualized to the client and according to age-specific needs.
Perform duties within the scope of physical therapy practice guidelines and in accordance with productivity standards.
Develop and revise a plan of care based on the initial assessment, continual reassessment of client needs, and expected outcomes.
Perform or monitor the application of treatments involving physical agents such as heat, water, and electricity.
Follow utilization guidelines and protocols.
Create and implement appropriate teaching plans and communicate clinical findings to physicians and involved personnel.
Document all skilled and medically necessary care thoroughly, timely, and accurately.
Maintain compliance with Medicare, state, federal, and insurance requirements as well as local policy.
Ensure client and organizational confidentiality is maintained at all times.
Participate in quality management programs and complete all required in-service programs.
Conduct supervisory visits and prepare updates according to local policy.
Supervise Physical Therapy Assistants in accordance with regulations and agency policy.
Participate in staff meetings, case conferences, and customer service programs as required.
Deliver a high level of customer service to all internal and external customers.
Demonstrate leadership competencies through communication, relationship building, and teamwork.
