Legacy Healthcare Services is searching for an experienced, motivated, and compassionate Physical Therapist (PT) to join our team in Greensboro, NC. This unique hybrid opportunity combines on-site facility care at Spring Arbor of Greensboro with personalized home-based therapy through our Better at Home program across Guilford and Rockingham Counties.

 

In this role, you will work directly with senior adults in both a vibrant community setting and in the comfortable environment of their own homes. With private, functional-based, 1-on-1 treatment sessions and no group setting constraints, you will have the autonomy to individualize care plans, build deep clinical relationships, and help patients live their best lives with purpose, safety, and joy.

 

Key Responsibilities

  • Comprehensive Clinical Care: Assess, plan, and deliver high-quality physical therapy services according to physician orders and the State Physical Therapy Practice Act.
  • Dual-Setting Treatment: Provide individualized, 1-on-1 rehabilitation treatments on-site at Spring Arbor of Greensboro as well as in patient homes throughout Guilford and Rockingham Counties.
  • Environmental & Functional Assessments: Evaluate home environments, muscle strength, gait, mobility, and ROM potential to build tailored rehabilitative care plans.
  • Caregiver & Family Guidance: Educate patients, families, and caregivers on home exercises, safe mobility strategies, and the proper use of therapeutic appliances.
  • Interdisciplinary Collaboration: Participate in clinical conferences, coordinate integrated care plans with multidisciplinary teams, and identify referrals for additional home health (PCA) services as needed.
  • Documentation & Leadership: Maintain progress summaries based on goal attainment and supervise Physical Therapist Assistants (PTAs) in compliance with state regulations and company standards.

Position requirements:
  • Degree in Physical Therapy from an accredited program.
  • Current Physical Therapy license in the state of North Carolina (or eligibility to obtain prior to start date).
  • Excellent clinical, interpersonal, and fiscal management skills.
