Physical Therapists to join our growing
Home Health team serving patients throughout Southern Fairfield County, CT. We currently have two territory-based opportunities available:
- Greenwich & Stamford
- Darien & New Canaan
As a Home Health Physical Therapist, you will provide skilled, one-on-one therapy services to patients in their home environments, helping them regain independence, improve mobility, and safely recover following illness, injury, surgery, or hospitalization.

Position OverviewThe Home Health Physical Therapist provides skilled physical therapy services under a physician-directed plan of care for patients receiving home health services. This role focuses on functional rehabilitation, patient safety, and improving overall quality of life while allowing clinicians the autonomy to manage their own schedules and caseloads within their assigned territory.
Key Responsibilities- Perform evaluations, reassessments, start-of-care visits, and discharges
- Develop and implement individualized treatment plans based on patient goals and functional needs
- Provide therapeutic interventions focused on mobility, strength, balance, endurance, and fall prevention
- Educate patients and caregivers on home exercise programs, safety, and disease management
- Collaborate with nurses, physicians, occupational therapists, speech therapists, and interdisciplinary care teams
- Complete timely and accurate clinical documentation, including OASIS assessments
- Monitor patient progress and adjust treatment plans as appropriate
- Support patients in achieving maximum independence within their home environment
Qualifications- Bachelor's, Master's, or Doctorate degree from an accredited Physical Therapy program
- Current Connecticut Physical Therapist license
- Minimum of 2 years of clinical experience preferred
- Home health experience strongly preferred
- Strong clinical assessment, communication, and organizational skills
- Ability to work independently and manage a flexible field-based schedule
- Reliable transportation and valid driver's license required

Compensation:Th pay range for this position is $90,000 to $110,000 annually + incentive plan and represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
