This position includes a $5,000 sign-on bonus!!!

Schedule: Part Time to Full Time hours - 32-40 hours/pay period - Monday to Friday - 8 a.m. to 4:30 p.m.

As a Physical Therapist, you will:
  • Deliver Direct Client Care - Conduct comprehensive evaluations and develop personalized treatment plans with short-term and long-term goals.
  • Maintain Accurate Client Records - Document services accurately and timely using Vivie's EMR system, ensuring compliance with all payer sources and authorization requirements.
  • Facilitate Follow-Up Programs - Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes.
  • Collaborate and Communicate Effectively - Work closely with clients, families, physicians, and the interdisciplinary team to provide care.
  • Ensure Regulatory Compliance - Complete and document supervisory visits with PTAs and RA/CNAs in accordance with state and regulatory guidelines.
  • Other Duties as Assigned - The duties and responsibilities listed above are representative of nature and level of work assigned and are not necessarily all inclusive.

This job also requires:
  • Current Physical Therapist license in the state of Minnesota.
