Physical Therapist to join our growing team in Worcester, MA. If you're an experienced PT looking for

As a home health PT, you'll have the opportunity to make a meaningful impact by helping patients improve mobility, regain strength, increase independence, and safely remain in their homes.
What You'll Do- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
- Provide therapeutic interventions to improve strength, mobility, balance, coordination, and functional independence.
- Help patients safely achieve their mobility and functional goals in their home environment.
- Educate patients and caregivers on exercises, mobility strategies, fall prevention, and appropriate use of equipment.
- Collaborate with patients, families, physicians, and interdisciplinary team members to coordinate care and achieve optimal outcomes.
- Monitor patient progress and adjust treatment plans as needed.
- Complete timely and accurate clinical documentation in accordance with company and regulatory requirements.
- Maintain current clinical knowledge through continuing education and professional development.
What We're Looking For- Current Massachusetts Physical Therapist license.
- Home health or outpatient experience preferred.
- Strong clinical assessment and treatment skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and effectively manage a patient caseload.
- Strong organizational and time-management skills.
- Valid driver's license and reliable transportation.
