Qualifications

• Master's degree in physical therapy from a CAPTE accredited program.
• Minimum of nine months of experience as a physical therapist.
• Preferred experience in community or home health settings.
• Current state license to practice physical therapy and compliance with professional standards.
• Valid CPR certification required while in clinical role.
• Licensed driver with ability to travel to various locations.

Responsibilities

• Provide patient care and document treatment per physician orders and agency policy.
• Perform evaluations and select diagnostic materials to assess patient needs.
• Communicate assessment results and recommended interventions to the physician before care starts.
• Develop and document tailored treatment plans with appropriate strategies and priorities.
• Deliver organized and efficient patient treatment, including education and counseling.
• Present relevant patient information to physicians and keep the team updated on treatment plans.
• Administer treatment in line with patient needs and agency parameters.
• Set objective and measurable goals collaboratively with team members and patients.
• Meet agency productivity standards and manage work hours effectively.
