What You'll DoAs a Home Health Physical Therapist, you will:
- Evaluate patients' mobility, strength, balance, and functional abilities in their homes
- Develop individualized treatment plans based on each patient's needs and personal goals
- Provide hands-on therapy to improve movement, safety and independence
- Adjust treatment plans as patients progress and their needs change
- Educate patients and caregivers on exercises, equipment, fall prevention and safe mobility
- Collaborate with physicians, nurses, therapists and other members of the care team
- Guide support staff in helping patients achieve their functional goals
- Complete accurate and timely clinical documentation
- Serve as a trusted therapy resource for patients, families, colleagues and the community
What You'll Need- Graduate of an American Physical Therapy Association-approved physical therapy program
- Current, unrestricted Physical Therapist license in the state of practice
- Dependable vehicle, valid driver's license and current automobile insurance
- Ability and willingness to travel throughout the assigned coverage area
- Ability to work independently while remaining connected to a collaborative care team
- Sit, stand, bend, lift, and move intermittently, and be able to lift 50-100 lbs.
