Position Summary

We are looking for a dedicated Physical Therapist (PT) who is passionate about geriatric care and wants to work in a facility that truly supports the "heroes who provide the care."

Full-time benefits start on the first day of the month following your start date!

Posted Salary Range

USD $52.00 - USD $52.00 /Hr.

Wage Commensuration

The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.

Duties & Responsibilities

As a Physical Therapist on our team, you will be the lead architect of resident mobility and recovery:
  • Evaluation & Treatment: Conduct comprehensive initial assessments and develop individualized, evidence-based treatment plans for residents.
  • Functional Restoration: Provide high-quality therapy focusing on gait training, balance, strength, and fall prevention.
  • Interdisciplinary Collaboration: Work closely with our Nursing leadership (DON/ADON), Occupational Therapists, and Speech Therapists to provide holistic, person-centered care.
  • Documentation: Maintain timely and accurate clinical records using Matrix, ensuring compliance with Medicare/Medicaid and state guidelines.
  • Education: Provide training to residents, families, and restorative nursing staff on safe transfer techniques and equipment use.


Qualifications & Requirements

  • Must be a graduate of an accredited Physical Therapy program.
  • Must have one year experience as an Physical Therapist (geriatric rehabilitation setting preferred)
  • Must have current license to practice as a Physical Therapist in facility-specific state
  • Must have current certification in Basic Life Support.
  • Reliable transportation required.
