What You'll Do- Carry out treatment plans established by the supervising Physical Therapist.
- Provide therapeutic exercises, gait training, balance training, transfer training, and functional mobility interventions.
- Monitor patient progress and communicate changes to the supervising PT.
- Educate patients and caregivers on exercises, safety techniques, and home programs.
- Collaborate with nurses, therapists, physicians, and the interdisciplinary care team.
- Complete accurate and timely documentation.
What We're Looking For- Active Minnesota Licensed Physical Therapist Assistant (PTA) license.
- Excellent communication and patient education skills.
- Passion for helping patients improve mobility and independence.
- Home health experience is preferred but not required.
- Valid driver's license and reliable transportation.
