Physical Therapist to join our dedicated healthcare team. This position offers a unique blend of administrative oversight and hands-on patient care, presenting an excellent opportunity for a PT looking to expand their professional scope.
Position: Field Physical Therapist
Key Responsibilities:- Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
- Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
- Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
- Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
- Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.
Qualifications:- Active license/registration with the Physical Therapy Board of California.
- Graduate of an accredited Physical Therapy program.
- Strong documentation and organizational skills.
- Excellent communication and interpersonal abilities.
- Willingness to travel to patient homes for evaluations and treatment.
- Flexibility regarding work locations throughout our service area.

Requirements:- Valid California Physical Therapy license.
- Current CPR certification.
- Valid California driver's license.
- Current California auto insurance.
- Completed physical examination, TB test, and required vaccines/titers.
- Two professional references.
