This caregiver optimizes medication therapy for complex hospitalized patients through clinical rounds, medication management, evidence-based recommendations, and collaboration with physicians and healthcare providers. Additional responsibilities include educating pharmacy residents and learners, participating in medication safety and stewardship initiatives, contributing to research and quality improvement projects, and supporting clinical protocol development. This role includes staffing responsibilities approximately every ninth weekend. A caregiver in this position will work Monday through Friday from 7:00am to 3:30pm with staffing requirements for weekends occurring every 9th weekend. No holidays or on-call expectations required. A caregiver who excels in this role will: Routinely and continually provide pharmaceutical care for patients, considering their age and clinical condition. Provide general and patient specific drug information to patients, physicians, nurses and, and other health care professionals. Support Clinical Pharmacists in providing pharmaceutical care to patients by direct interaction and formal educational programs. Maintainexpertisein areas of practice through continual learning, journalreviewand other educational activities. Educate Clinical Pharmacists, Technicians, Pharmacy Students, Post-GraduateTraineesand other caregivers in Pharmacotherapywithin inthe specialty area. Minimum qualifications for the ideal future caregiver include: PharmD orBachelors Degree in Pharmacyfrom an accredited college of Pharmacy. If hired for a specialty area where specialty residency training is available, then specialty residency is required Current state licensure as a Registered Pharmacist (RPh) or eligible within90 daysof hire CurrentBasic Life Support (BLS) Certification through the American Heart Association (AHA) andcompletion of a Board of Pharmacy approved course inthe administration of immunizations or certificationobtained within90 daysof hire (if administering immunizations) Completion of a Post-Graduate Year 2 Residency (PGY2) or fellowship OR completion of a Post-Graduate Year 1 Residency (PGY-1) and two years of experience as a Clinical Specialist or an equivalent role, including providing pharmaceutical care and performing pharmacotherapeutic monitoring for at least 50% of time in the specialty area OR four years of experience as a Clinical Specialist or an equivalent role, including providing pharmaceutical care and performing pharmacotherapeutic monitoring for at least 50% of time in the specialty area Preferred qualifications for the ideal future caregiver include: Board certification PGY2 Residency training in Internal Medicine, Acute Care, or Geriatric Medicine Experience participating in clinical rounding within hospitalist or internal medicine team Physical Requirements: Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time. Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs. Position often includes reaching, stretching, stooping, climbing steps and sitting. Must have normal or corrected vision. Must have ability toclearly communicate verbally by phone or in person Personal Protective Equipment: Follows standard precautions using personal protective equipment as required.
