The
Ideal candidate is a licensed pharmacist with a
Pharm.D. or Bachelor's degree in Pharmacy from an accredited college. They demonstrate strong clinical knowledge and a commitment to patient-centered care in an ambulatory oncology setting. While
hospital pharmacy experience (6 months or more) is preferred, the candidate should excel in accuracy, communication, and collaboration within a multidisciplinary team.
Working Hours: The working hours are 9:30am-6pm Mon-Fri

- Communicate promptly with providers regarding prescription changes, patient concerns, medication-related issues, or other clinical needs as appropriate.
2. Clinical Assessment & Patient Counseling- Conduct comprehensive patient assessments, including:
- Review and maintenance of an updated home medication list.
- Assessment of medication adherence.
- Evaluation of patient quality of life and therapy outcomes.
- Assess the appropriateness of specialty medications and monitor patients throughout therapy to support achievement of desired clinical outcomes.
- Provide specialty medication counseling and education, including:
- Medication administration.
- Expected therapeutic outcomes.
- Potential adverse effects and their management.
- Participate in specialty clinical consultations and collaborate with healthcare providers to optimize patient care.
3. Documentation & Regulatory Compliance- Accurately document current and relevant patient information related to specialty medication therapy within the electronic health record (EHR).
- Complete and maintain initial patient assessments and ongoing reassessments in compliance with organizational policies and accreditation standards, including URAC requirements.
- Maintain thorough and accurate documentation of all patient interactions, including but not limited to:
- Telephone communications
- Emails
- In-person pharmacy visits
- Clinical consultations
- Ensure all documentation is timely, complete, and compliant with applicable regulatory and accreditation standards.

Licenses and Certification- Required: RPH - Registered Pharmacist Texas State Board of Pharmacy. within 90 Days and
- Required: BLS - Basic Life Support American Heart Association (AHA). within 30 Days and
