Full Job Description
General Information:

Job title: Staff Pharmacist - Hospital Pharmacy

Schedule: Full-time, 80 hours per pay period; Monday-Friday, scheduled between 7:00 AM - 4:30 PM

Weekend rotation: Every 4th weekend; 7:00 AM - 3:00 PM

Holiday rotation: Every 4th holiday

Position Summary:

The Staff Pharmacist performs clinical and professional pharmaceutical work in the compounding and dispensing of drugs and IV supplies.

Position Responsibilities:
  1. Interpret physician's order from direct copy or via electronic provider order entry.
  2. Coordinate proper scheduling of the administration of medication with nursing service.
  3. Provide clinical drug information to medical and nursing staff.
  4. Maintain patient profiles and monitor patient medications.
  5. Make decisions in selection of drugs and use of drugs including use of formulary and formulary substitutions.
  6. Participate in inservice activities (teaching).
  7. Compound prescriptions and IV admixtures. Supervises supportive personnel.
  8. Dispense prescriptions using acceptable and good pharmaceutical technique.
  9. Responsible for following departmental policies and procedures.
  10. Supervises interns, externs, pharmacy technicians, and other support personnel in the absence of or as designated by the director.
  11. Actively participate in the Antibiotics Stewardship program
  12. Maintains a working knowledge of the MBMC antibiogram.
  13. Maintains a working knowledge of the spectrum of activity of formulary antibiotics.
  14. Work to affirm the appropriate use of antibiotics in all situations.
  15. Manage antibiotic de-escalation - IV to PO, C&S response, etc.
  16. Lead the antibiotic pause.
  17. Lead efforts to reach goals of therapy for each patient we work with.
  18. Performs other related duties as required including computer database maintenance for pharmacy systems and automated dispensing units, pump library maintenance and records of activities.
  19. Actively provides clinical pharmacy services to the patients of Mile Bluff Medical Center
  20. Renal dosing services.
  21. Pharmacokinetic services.
  22. Monitoring of the effect of medications including reviewing and responding to affected lab values, reviewing and managing ADR's, etc.
  23. Provides medication reconciliation at the time of admission, transfer, and discharge as appropriate for each patient.
  24. Provides medication education to patients, caregivers, and professional staff.
  25. Participates in the department's DUE activities with data collection when needed.
  26. Coordinates the Formulary file updating in the pharmacy computer system and the facility's drug database on an ongoing basis.
  27. Supervises and trains intern-pharmacists.
  28. Participates in in-service training.

Position Requirements:
  • Professional Pharmacy degree required. Doctorate preferred.
  • Must hold and maintain current Wisconsin Pharmacist license.
  • BLS certification required.
  • 1+ years related work experience required.
  • Exceptional accuracy and attention to detail required.

  • Intermediate to expert proficiency with computers is required.
  • Requires assessment, evaluation, and decision-making skills.
  • Must possess exceptional clinical skills.
  • Strong quantitative and analytical competency.
  • Familiarity with pharmacy law.
  • Self-starter with excellent interpersonal communication, oral and written communication, and problem-solving skills.
