What Are We Looking For?

Your Health is currently looking for a Pharmacist to join our growing urgent care family. The Clinical Pharmacist is a specialized healthcare professional who works as a part of the care team to optimize patient outcomes through the safe and effective use of medications. The Clinical Pharmacist will provide patient consultation, recommendations, and education in accordance with the patient's plan of care. A successful Clinical Pharmacist will be able to perform these essential duties and responsibilities. Review records of patients to determine the appropriateness of medication therapy. Evaluating the patients' condition to ensure all issues are being addressed and/or treated. Develop an effective medication recommendation that minimizes the risk of adverse side effects and ensure appropriate drug therapy. Counsel patients on appropriate medication use and assess patient for barriers to medication adherence in an effort to prevent hospitalizations. Collaborate with healthcare professionals to ensure optimal patient care. Accurately document medication plans and patient progress in the medical record, as well as ensuring proper billing of visits. Interact with patients via phone, email, patient portal, telephone, text, in person, etc. to gather necessary information.
