WI-SSM Health St. Mary's Hospital - Madison Worker Type: Regular Job Highlights: This is a full time night shift Pharmacist position for St. Mary's Hospital in Madison working a 7 on/7 off schedule. The hours are 9:00 p.m. to 7:00 a.m. Department: Inpatient Hours: 70 hours in a two week period Schedule: 7 on/off, 9:00 p.m. to 7:00 a.m. Additional differentials are available for night shift and weekends. A one-time sign on bonus may be available to qualified applicants with a 2 year service commitment. Job Summary: Provides pharmaceutical care to patients, verifies medication orders, compounds and dispenses pharmaceuticals in accordance with federal, state and local regulations and hospital policy. Serves as a resource to other healthcare professionals including pharmacy students and residents and ensures that patient care meets standards of safety and effectiveness. Oversees the duties of the pharmacy technicians and interns. Job Responsibilities and Requirements: PRIMARY RESPONSIBILITIES Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable. Documents medication related plans and interventions in electronic medical record. Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners. Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration. Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients. This includes supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system. Precept pharmacy students and residents, as applicable. Works in a constant state of alertness and safe manner. Performs other duties as assigned. EDUCATION Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy EXPERIENCE No experience required PHYSICAL REQUIREMENTS Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements. Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors. Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc. Frequent keyboard use/data entry. Occasional bending, stooping, kneeling, squatting, twisting and gripping. Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs. Rare climbing. REQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment. Other resuscitation certifications may be required dependent on department.
