Full Job Description
Pharmacist - Full-Time (7 On / 7 Off)

Black River Health is seeking a dedicated full-time Pharmacist to join our collaborative, patient-centered Pharmacy team. This benefited, day-shift position features a highly desirable 7-day on / 7-day off schedule, giving you built-in extended time off every other week-including every other weekend.

We're looking for a pharmacist who thrives in an acute care environment and enjoys practicing at the top of their license. If you have Cerner EHR experience, strong IV preparation skills, and recent acute care practice, you'll feel right at home here. Essential Duties:

  • Provide comprehensive clinical pharmacy services to optimize medication therapy through evaluation of drug selection, dosing, route, duration, and monitoring parameters.
  • Serve as a medication therapy expert by advising providers and clinical staff on pharmacotherapy decisions, drug efficacy, toxicities, contraindications, interactions, and cost-effective alternatives.
  • Design and adjust patient-specific medication regimens using pharmacokinetic and pharmacodynamic principles and patient-specific factors to optimize therapeutic outcomes.
  • Manage pharmaceutical inventory processes, including procurement, storage, rotation, distribution, regulatory compliance, formulary adherence, and cost-containment strategies.
  • Lead and participate in medication safety, quality improvement, and incident investigations by conducting root cause analyses, identifying system issues, and implementing sustainable corrective and preventive measures.
