Full Job Description
Responsibilities:
  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client's unique needs
  • When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
  • Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:
  • Current Certificate of Competence with the College of Nurses of Ontario as RN.
  • Availability to work FT Days, Evenings, Overnight Shift and every other weekend.
  • Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
  • A willingness to attain required SE Health certifications
  • A current CPR certificate is required.
  • This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
  • Required to travel to Guelph/Fergus/Elora/Rockwood and surrounding areas.
  • Trach and Ventilation experience (preferred)

We use AI to take notes during our interview. All applications and interviews are reviewed by our Talent Acquisition team. This role is a new addition. The hiring pay range is $37.35-41.35 per hour, based on experience.
