Job DescriptionThe RN Care Manager is an experienced Registered Nurse who serves as a core member of a multidisciplinary team, facilitating communication and coordination of patients' healthcare needs among primary care providers, specialty providers, healthcare facilities, community resources, and payers. The RN Care Manager is responsible for coordinating and supporting care for pediatric patients with complex medical and/or psychosocial needs within pediatric clinics, while overseeing transitions of care and facilitating referrals to specialty services, clinically based programs, and community resources.
Through comprehensive care management, the RN Care Manager assesses patient and family needs, develops individualized care plans, promotes patient and family engagement and self-management, identifies and addresses barriers to care, and monitors progress toward established health goals. The RN Care Manager collaborates closely with patients, families, and the healthcare team to ensure timely access to services, improve care coordination, and support optimal health outcomes across the continuum of care.
Required Qualifications:- Coursework/Training: Graduate from an accredited school of Nursing
- Upon hire: Washington Registered Nurse License
- 3 years clinical nursing experience.
- 1 year Care Management or similar experience.
Preferred Qualifications:- Associate's Degree in Nursing
- Bachelor's Degree in Nursing

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
