Patient Placement Coordinator RN located in Anaheim, CA. This position will work Full Time 10-hour Variable shifts.
The Patient Placement Center Nurse supports care for Providence St Joseph Health (PSJH) patients by evaluating and coordinating the patient intake process. This position requires advanced clinical judgment, rapid decision-making, and the ability to independently assess all patient transfer requests, including those that are high-acuity, in a time-sensitive environment. Responsibilities include taking and triaging calls and facilitates inbound and outbound transfers for Providence facilities. The Patient Placement Center Coordinator Nurse acts as a liaison between physicians, hospitals and other care providers who seek to refer, schedule admission or transfer patients into the hospital. Ensures timely medical communication for patient referrals and transfers to/from the facility. Employs critical thinking to integrate objective data with knowledge gained from preliminary/initial information with the ongoing assessment process, including the objective and subjective experiences and patient health care history for optimal placement. In addition to core duties, the Nurse serves as a clinical resource for the team, providing guidance on clinical concerns, level of care determination, and regulatory considerations. The RN supports onboarding and training of new staff, helping reinforce consistent practices across the teams. This role supports all transfer center operations, including but not limited to the Southern, Northern, and Behavioral Health California Transfer Centers. Providence's centralized Transfer Center coordinates patient transfers across the organization's regional, divisional, and system healthcare network.
Required Qualifications:
- Associate's Degree - Graduate from an accredited Associate Degree in Nursing (ADN)
- California Registered Nurse License upon hire
Preferred Qualifications:
- Bachelor's Degree - Graduate from an accredited School of Nursing. Bachelor's Degree in Nursing.
- Minimum of 2-5 years nursing experience.
- 1 year of Working knowledge of Epic and Bed management system.
- 1 year of Call center experience.
Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
