Position Summary

The Licensed Clinical Social Worker (LCSW) independently provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems. The LCSW incorporates concepts and theories associated with life span development, neurobiology, and behavioral change. The LCSW will collaborate with the patient and support system to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter/Multi-Disciplinary Team (IDT). The LCSW shall perform duties in a manner that respects patient self-determination, that reduces health disparities/barriers to care, and adheres to the NASW code of ethics, Standards of Practice (SOP), and mission.

MAJOR CHALLENGES:
- Working with diverse populations and their support systems to provide services unique to each population.
- Analyzing, interpreting and integrating interview data and observational information to develop the most appropriate service/treatment approach.
- Supporting family and/or other support systems and patients in finding appropriate community services when resources are limited.
- Providing expertise and guidance to patients, their significant others, staff members and department/program leaders in all matters of social importance to the population.
- Maintaining knowledge and skill base in a rapidly changing and pluralistic medical and healthcare environment.
- Communicating consistently and effectively with all levels: patients, family members, Inter/Multi-Disciplinary Team (IDT), physicians and other health care providers.
- Integrating and coordinating the work process and information across the continuum of care.
- Identifying and responding to changes in policies, regulations and procedures that can impact the patient/client.
- Work independently with the ability to determine when to seek appropriate consultation.

Patient Population Served: Neonate, Pediatric, Adolescent, Adult, Geriatric
Consequently, the employee must be competent in the interpretation of a patient's self-report or behavior, and this information must be interpreted with an understanding of the cognitive, physical, emotional/psychosocial and chronological maturation process. The treatment of patients should be individualized and should reflect an understanding of the developmental needs and range of treatment needs for each patient.

Position Specific Responsibilities

  • Assessment: Independently provides strength based comprehensive biopsychosocial, emotional, spiritual, and cultural assessments.
    • Assess for and recognize social determinants for wellness and recovery.
    • Conducts ongoing assessment of patients appropriate to age and cultural background in accordance with patient directed goals.
    • May include advance care and end of life discussion as appropriate to setting.
    • Intervention: In the context of a clinical relationship, provides social work interventions to support and empower patients and their family/support systems to effectively cope with the impact of medical and/or mental health conditions, optimize functioning and improve overall well-being.
    • Formulates, implements, and monitors a social work plan of care that supports and identifies therapeutic solutions or adjustments to medical, social, emotional and financial problems which may impact health or well-being.
    • Care plans are patient-centered, strength-based, and culturally-relevant.
    • Utilizes counseling, motivational interviewing, crisis intervention, education, case management, etc. as appropriate and required to the clinical setting.
    • Diagnosis: As appropriate and required to the clinical setting, perform diagnostic evaluation of psychiatric and substance use disorders.
    • Establish a diagnosis based upon DSM and ICD diagnostic criteria.
    • Treatment: Incorporate evidence based theories and methods, provide specialized clinical treatment services as appropriate and required to the clinical setting.
    • Utilizes the Professional Edition: Current Procedural Terminology (CPT) for psychiatric and substance use disorders to resolve and/or reduce signs and symptoms of diagnosis as appropriate and required to the clinical setting.
    • Case Management: Provides effective case management including triaging patient needs, adapting approaches to meet additional needs, and collaborating with internal and external resources as appropriate.
    • Coordination of Care: Creates strength-based, collaborative care planning with the patient and their family/support systems.
    • Develops and maintains effective working relationships with various departments, community health, social service agencies, payors and other partners.
    • Advocates for support appropriate to the age, biopsychosocial needs, cultural, spiritual, and health literacy of the patient.
    • Identifies barriers to continuing care and planning of next level of care.
    • Functions within the existing care team model.
    • Collaborates with the inter/multi-disciplinary team facilitating care conferences for complex patient care coordination.
    • Documentation: Maintains timely, written documentation as well as verbal handoffs within professional standards and in accordance with department policies and procedures.
    • Submits relevant ICD/CPT and DSM codes for reimbursement as appropriate and required to the clinical setting.
    • Advocacy: Acts as an advocate for the needs and interests of the patient and their family/support system by supporting self-determination and autonomy.
    • Advocates for navigation of complex healthcare systems, various payers, community services, education, system changes, etc.
    • Adheres to mandatory reporting of abuse, neglect, and/or exploitation.
    • Communication: Provides clear and concise communication through consultation, education, facilitation and team organization to patients and their family/support system.
    • Collaborates with internal and external partners, and patient caregivers to enhance provision of care and services in care settings across the continuum.
    • Communicates changes in patient condition or goals of care related to medical, mental, social, spiritual, environmental, and eligibility factors to the inter/multidisciplinary team.
    • Contributes to and updates the inter/multi disciplinary care plan.
    • Performs duties in a manner that adheres to social work professional standards and code of ethics and in compliance with regulatory bodies; NASW (National Assoc.
    • Social Workers), TJC (The Joint Commission), etc.
    • Protects confidentiality and establishes professional boundaries.
    • Maintains a constant awareness of safety and legal aspects of the position.
    • Educates and communicates with the inter/multi-disciplinary team.
    • This position works closely with the Palliative interdisciplinary team to provide clinical counseling, assessment, case management, crisis intervention, and psycho-education with multi-stressed patients and families relevant to the dying process, end of life care planning and bereavement.
    • The Providence Beginnings team provides social work, case management, and home visiting nurse services to high risk maternity patients across the continuum of care.
    • Social workers are assigned to cover outpatient clinics and the hospital maternity and neonatal intensive care units.
    • The Providence Beginnings social worker, in conjunction with the patient and caregivers, develops, implements, and monitors a plan of care to support positive birth outcomes and successful transition to parenting.


Education and/or Experience
  • Required - Master's Degree Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE).
  • Required LCSW
  • Home health/hospice or palliative/ambulatory care experience preferred.
  • Required - 2 years Relevant Social Work experience.


Certifications, Licenses, and Registrations
  • Required upon hire: California Clinical Social Worker License (Vendor Managed)


Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range:$41.41 - $64.29

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
