Job Summary & Requirements

The Palliative Care Clinical Coach is a regionally focused, Ontario Health-funded role responsible for leading the implementation of palliative care priorities across hospital, community, hospice, and regional care settings. The position operates at a program and system level and functions as a clinical implementation lead and project resource, accountable for translating provincial frameworks, funding requirements, and RVH strategic direction into actionable work plans, measurable deliverables, and sustainable improvements in care delivery.

This role is directly aligned with RVH's Clinical Priorities Plan, specifically the initiative to enhance regional palliative and end-of-life care through the development of a Regional End of Life Care Program, and supports the broader organizational direction reflected in Vitally You. The position plays a key role in operationalizing a phased roadmap focused on system optimization, education and workforce development, integration of models of care, improved care coordination, and long-term program development.

A critical component of this position is contributing to and sustaining a positive, inclusive, and supportive team culture. The position is a hybrid role, with the majority of work performed remotely, regular on-site presence, and occasional travel across the region. The role requires flexibility, adaptability, and the ability to work effectively across both virtual and in-person environments while maintaining strong relationships and a positive team dynamic.

This position also carries accountability for Ontario Health funding requirements, including development of co-designed work plans, documentation of activities, data tracking, and reporting to demonstrate achievement of deliverables. All work is focused on implementation, practice change, capacity building, and system integration.

Education:

I. Degree in a regulated health discipline (e.g., RN, NP, RSW or equivalent) required
II. Specialized training in palliative care (e.g., CAPCE or equivalent) required
III. Graduate degree (e.g., MScN, MSW or equivalent) considered an asset

Experience:
  • Minimum 5 years progressive experience in palliative care across hospital and/or community settings
  • 3-5 years of experience in program and project management, including planning, executing, and evaluating initiatives aligned with provincial frameworks and funding requirements.
  • 3-5 years of demonstrated change management experience to support practice transformation, adoption of models of care, and implementation across hospital and community environments.
  • 3-5 years of experience in clinical education, coaching, and facilitation, including designing and delivering education programs, mentoring interprofessional teams, and supporting competency development.
  • 3-4 years of experience in quality improvement to lead initiatives focused on improving care consistency, patient experience and system integration.
  • 3-5 years of strong stakeholder engagement and partnership development skills to collaborate effectively with internal teams.
  • 2-3 years of experience in data collection, performance measurement, and reporting to support accountability for funded deliverables and to demonstrate program outcomes and appropriate use of funds.
  • Experience leveraging existing processes, partnerships, and organizational initiatives to support effective and efficient implementation
  • Experience working with diverse contributors and adapting approaches to meet a wide range of needs and environments
  • Experience working with Indigenous partners and communities is considered an asset, including an understanding of culturally safe and trauma-informed approaches to care
  • Demonstrated experience contributing to strategic planning and execution, including aligning initiatives with organizational priorities, translating strategy into operational plans, and supporting implementation of regional or system-level programs

  • Driving system-level practice change to support adoption of standardized models of care across hospital and community settings
  • Leading and supporting quality improvement initiatives to address identified system gaps and improve patient outcomes
  • Conducting education needs assessments and delivering training and coaching to build provider competencies
  • Providing clinical coaching, mentorship, and consultation to interprofessional teams
  • Facilitating collaboration and integration across hospital, community, and hospice partners
