Job Summary:

We are seeking a compassionate and patient-centered Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing adult base primary care team. The ideal candidate will provide comprehensive, high-quality care to patients across the lifespan, focusing on health promotion, disease prevention, and chronic disease management in an outpatient setting.

Position Details:
  • Provide comprehensive primary care services, including assessment, diagnosis, and treatment of acute and chronic conditions to patients 14 years and above
  • Perform physical examinations and interpret diagnostic tests and lab results
  • Develop and implement individualized treatment plans
  • Prescribe medications in accordance with state regulations and clinical protocols
  • Deliver preventive care, including wellness exams, immunizations, and health screenings
  • Educate patients and families on disease prevention, health maintenance, and treatment plans
  • Manage chronic conditions such as diabetes, hypertension, and asthma
  • Collaborate with physicians and interdisciplinary care teams to ensure coordinated care
  • Maintain accurate and timely documentation in the electronic medical record (EMR)
  • Participate in quality improvement initiatives and population health management
  • Adhere to clinic policies, procedures, and compliance standards
