Job SummaryJob Details**Applicants must reside in Northeast OhioAs an Outpatient Pharmacist, a caregiver in this role will support a high-volume compliance packaging pharmacy processing approximately 1,300 prescriptions per day. This hybrid position is 75% remote following three months of training, with all work performed over the phone. The caregiver will rotate through five key functions over an eight-week period, including clinical order review for fulfillment, refill requests, prior authorization and clinical inquiries, medication therapy management (MTM) and onboarding new patients through medication synchronization.
A caregiver in this position will work full time, Monday through Friday, with no weekend, holiday or on-call requirements. Caregivers will work 8-hour shifts scheduled between 7:00am and 6:00pm. The schedule will rotate through five different job responsibilities over an 8-week period. This is a hybrid position with one week onsite followed by seven weeks working from home at CCAC.A caregiver who excels in this role will:
- Provide pharmacy services and ensure the appropriate selection, dosing, administration and dispensing of medication orders.
- Provide safe, efficient, patient-centered, evidence-based medication services.
- Verify medication orders and/or dispense medications accurately according to state and federal regulations.
- Contribute to medication safety initiatives.
- Document services in a complete, accurate and timely manner.
- Ensure all pharmacy operations comply with national accreditation standards.
- Maintain professional interactions with patients to facilitate successful patient experiences.
- Educate and train students, residents, other caregivers, and/or patients.
- Be accountable for drug use policy adherence.
Minimum qualifications for the ideal future caregiver include:
- PharmD or Bachelor's of Science in Pharmacy from an accredited College of Pharmacy
- Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire
- Basic Life Support (BLS) Certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations within 90 days of hire for those administering immunizations
Preferred qualifications for the ideal future caregiver include:
- Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience preferred.
- Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area
- Board Certification
- Prior retail/community pharmacy experience
Physical Requirements:
- Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.
- Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 20 lbs.
- Position often includes reaching, stretching, stooping, climbing steps and sitting.
- Must have normal or corrected vision.
- Must have ability to clearly communicate verbally by phone or in person
