Completes documentation and record-keeping accurately and within expected time frames. Provides ongoing therapeutic lifestyle coaching using approved resources and manages assigned caseloads. Participates in practice changes resulting from performance improvement activities and supports departmental quality standards. Serves as an advocate for care with community agencies, including pharmacies, insurance companies, and employers. Educates patients and families about current research and assists with identifying and recruiting candidates for research studies. Seeks professional growth opportunities and participates in community activities such as educational lectures and support groups. Provides diabetes instruction in group and individual sessions according to American Diabetes Association standards and departmental policy. Obtains individual patient assessments, identifies learning barriers, and provides appropriate resource referrals. Establishes individualized learning goals and educational plans in collaboration with patients and families. Prescribes appropriate nutrition plans based on patient needs and diagnoses.
