full-time Occupational Therapist(s) to join our dedicated home health team with the added benefit of flexible scheduling and competitive pay.
Position Highlights:- Role: Full Time
- Locations: Eau Claire, WI area (including surrounding communities) & Wausau area
- Pay: Competitive starting at $40/hr
- Sign-on bonus: $5,000 for Eau Claire market
Essential FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
Patient Care- Completes an initial physical evaluation of patient as prescribed by the attending physician, including an assessment of rehabilitation prognosis based on functional status
- Submits clear and appropriate documentation in a timely manner and documents patient education in the medical record
- Communicates effectively with physicians, supervisors, and coworkers in a timely manner
- Develops a care plan, which establishes goals based on therapeutic, preventive, and rehabilitative therapy actions. Includes the patient and the family in the planning process and adapts the plan of care as patient needs change
- Complies with agency policies and procedures. Promotes and maintains an agency environment in compliance with federal, state, and local regulations
- Counsels the patient and family in meeting related needs
- Provides health care instructions to the patient as appropriate per assessment and plan of care
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient
Communication- Prepares clinical notes and updates the primary physician when necessary
- Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required
- Communicates with community health related persons to coordinate the care plan
Additional Duties- Ensures that arrangements for equipment and other necessary items and services are available
- Attend in-service training and mandatory agency meetings
- Maintains a professional appearance
Education and Experience- Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
- Registered with the National Registration Examination of the American Occupational Therapy Association
- Two years of clinical experience as an occupational therapist
- Excellent observation, verbal and written communication skills, and problem solving skills
- Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills
Skills, Knowledge and Abilities- Able to relate positively, effectively, and appropriately with patients, families, community members, and providers
- Able to read, write, speak and understand English
- Meets all health requirements, as required by law
- Demonstrates basic computer knowledge and ability with an aptitude to learn company applications
