  • Maintain accurate documentation in compliance with regulations
  • Monitor patient progress and adjust treatment plans as needed

Occupational Therapist (OT)
Lakeport Post Acute | Lakeport, CA

Location: Lakeport, CA
Pay: Starting at $70.00 per hour, DOE
Schedule: Full-Time and Part-Time Opportunities Available

Occupational Therapist Responsibilities
  • Evaluate residents and develop individualized treatment plans based on functional needs and goals
  • Provide occupational therapy services to improve independence with activities of daily living (ADLs)
  • Implement evidence-based therapeutic interventions and adaptive strategies
  • Assess cognitive, physical, and functional abilities to maximize patient outcomes
  • Collaborate with physicians, nurses, therapists, and interdisciplinary team members
  • Educate residents, families, and caregivers on treatment plans, progress, and home exercise programs
  • Participate in care planning meetings and interdisciplinary team conferences
  • Recommend adaptive equipment and environmental modifications when appropriate
  • Maintain accurate and timely documentation in compliance with facility, state, and federal regulations
  • Monitor patient progress and modify treatment plans as needed
