Overview

HealthPro Heritage has a great Per Diem and Full-Time Occupational Therapist (OTR) Opportunities in our Long Term Care setting in Juneau, WI. 

  • PRN Wage Ranges:  $48-$52 per hour
  • Opportunity for both full-time and consistent PRN hours each week
  • Long Term Care Setting includes 
    • Short term rehab, Long-term residents, & Individuals with Intellectual Disabilities
  • Unique opportunity to assist in the subacute brain injury center! We have a very supportive team including: PT, OT, neuropsych, psychiatry, and physiatry. We have group treatments, co-treatments, household events and outings.  
Responsibilities
  • Patient Assessment: Patient Assessment: Evaluate clients' physical, emotional, and cognitive abilities using observations and standardized tools.
  • Treatment and Education: Create personalized treatment plans to meet clients' goals and enhance their functional abilities. Educate clients and their families on improving daily activities, including the use of adaptive equipment and environmental modifications.
  • Documentation: Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards.
  • Advocacy and Support: Advocate for clients' needs and support them in accessing community resources, services, and accommodations.
