Full Job Description
Occupational Therapist (OT) | Full-Time | Greenbrae, CA | $60 - $67/Hour

You will support high-quality occupational therapy services in a Skilled Nursing setting, helping residents regain function, independence, and confidence in daily life.

The Role

The Occupational Therapist is a licensed healthcare professional responsible for the evaluation, development, implementation, and oversight of individualized occupational therapy plans of care. This position is classified as Non-Exempt and reports to the Director of Rehabilitation and the Facility Administrator.

Core responsibilities include:
  • Conducting comprehensive occupational therapy evaluations and re-evaluations in accordance with physician orders and California standards of practice
  • Developing, implementing, and modifying individualized plans of care with measurable, function-based goals
  • Delivering skilled therapeutic interventions addressing ADLs, IADLs, neuromuscular re-education, upper extremity rehabilitation, cognitive retraining, sensory integration, visual-perceptual skills, and adaptive strategies
  • Assessing and recommending adaptive equipment, assistive devices, splints, orthotics, and environmental modifications to enhance independence and safety
  • Fabricating and training patients in the use of splints or adaptive devices as clinically indicated and within scope of practice
  • Monitoring patient progress and modifying treatment plans based on clinical response and objective outcomes
  • Reviewing patient medical history, conducting patient interviews, and observing functional task performance
  • Evaluating patient home or workplace environments and recommending modifications based on health and functional needs
  • Leading discharge planning and transition-of-care coordination to promote successful reintegration into home and community environments
  • Communicating evaluation findings, progress updates, and discharge outcomes to physicians, interdisciplinary team members, patients, and families
  • Educating patients, families, employers, and caregivers on how to accommodate and support the patient's functional needs
  • Supervising and directing Occupational Therapist Assistants in accordance with California state law and professional regulations
  • Providing appropriate oversight to rehabilitation aides, students, and volunteers as assigned
  • Completing timely, accurate, and compliant documentation consistent with federal, state, payer, and organizational requirements
  • Participating in care conferences, quality assurance initiatives, and collaborative planning as requested
  • Maintaining compliance with HIPAA, patient rights, OSHA, infection control, and all applicable California state practice standards
  • Treating all residents with dignity, respect, and compassionate professionalism

Physical requirements include the ability to assist with patient transfers, upper extremity handling and positioning techniques, mobility assistance, repositioning and supporting patients with or without assistive devices, prolonged standing, walking, bending, reaching, kneeling, and stair navigation. The ability to assist in resident evacuation during emergency situations is also required.

  • Graduate of an accredited Occupational Therapy program (Bachelor's, Master's, or Doctorate) - required
  • Successful completion of the National Board for Certification in Occupational Therapy (NBCOT) - required
  • Current CPR/BLS certification (or willingness to obtain per facility policy)
  • Ongoing compliance with continuing education requirements established by the California Occupational Therapy Board
  • Experience in a skilled nursing, post-acute, hospital, or related medical setting - strongly preferred
  • Excellent verbal and written communication skills with the ability to exercise sound clinical judgment
  • Composure and professionalism in dynamic, fast-paced clinical environments
  • Strong organizational skills and the capacity to make independent clinical decisions
