Pleasant Hill Post Acute
Location: Pleasant Hill, CA
Employment Type: Full-Time, Part-Time, or PRN
Pay:$60-$62+ per hour, depending on experience

Pleasant Hill Post Acute is seeking a compassionate, motivated, and dedicated Occupational Therapist to join our rehabilitation team. We offer Full-Time, Part-Time, and PRN opportunities for OTs who are passionate about helping patients regain independence and achieve their highest level of function.

What You'll Do
  • Evaluate patients and develop individualized treatment plans based on their functional needs and goals.
  • Provide skilled occupational therapy services to help patients improve independence with activities of daily living.
  • Help patients maximize functional abilities and improve their overall quality of life.
  • Monitor progress and adjust treatment plans as appropriate.
  • Collaborate with physicians, nursing staff, physical therapists, speech therapists, and other members of the interdisciplinary team.
  • Communicate effectively with patients, families, and caregivers regarding treatment goals and progress.
  • Complete accurate, timely, and compliant clinical documentation.
  • Provide compassionate, patient-centered care while maintaining professional standards.
