Occupational Therapist (OT) - Home HealthSummitWest Care | Mesa County, CO
Full-Time | Flexible Daytime Schedule
$48-55/hour DOE + Mileage ReimbursementResponsibilities:- Perform occupational therapy evaluations, reassessments, recertifications, and discharges
- Develop and implement individualized treatment plans focused on safety, function, and independence
- Assess patients' ability to performs ADLs
- Conduct home safety evaluations and recommend adaptive equipment or environmental modifications
- Educate patients and caregivers on home programs, mobility strategies, and safety techniques
- Monitor patient progress and adjust treatment plans as appropriate
- Collaborate with nurses, therapists, caregivers, physicians, and family
- Supervise and support COTAs when applicable
- Complete accurate and timely clinical documentation
Qualifications:- Current Colorado Occupational Therapist (OT) license in good standing
- Graduate of an accredited Occupational Therapy program
- Strong clinical assessment, communication, and documentation skills
- Ability to work independently in a home health environment
- Excellent time-management and organizational skills
- Valid driver's license, reliable transportation, and auto insurance
Home health experience is preferred but not required. We are willing to train the right candidate!
