What You'll Do- Conduct initial and ongoing occupational therapy evaluations in the home
- Assess patient functional status, including strength, endurance, coordination, communication, and self-care abilities
- Evaluate the home environment for safety risks and barriers to independent living
- Develop, implement, and update individualized plans of care in collaboration with the Health Care Provider and RN
- Complete initial evaluations, treatment notes, and re-evaluations at least every 30 days
- Complete comprehensive assessments and OASIS documentation accurately and on time
- Observe, document, and report patient progress and significant changes; obtain updated orders as needed
- Identify and recommend adaptive equipment and strategies to improve independence with activities of daily living (ADLs)
- Educate patients and caregivers on adaptive techniques and devices
- Provide supervision and support to Occupational Therapy Assistants (OTAs), as assigned
- Collaborate with physicians and interdisciplinary team members to deliver high-quality care

Qualifications- Bachelor's degree from an accredited Occupational Therapy program
- Active Occupational Therapist license in the state of practice
- Minimum of one (1) year of OT experience; home health experience preferred
- Valid driver's license, reliable transportation, and auto insurance
- Ability to complete home visits and document care accurately and timely
- Home health documentation experience preferred (Home Care Home Base a plus)
