Responsibilities
- Evaluate and treat patients in the home setting
- Develop individualized plans of care
- Provide therapeutic interventions and functional training
- Educate patients and caregivers
- Complete EMR documentation
Requirements
- Active NY license as an OT
- Degree from an accredited OT program
- Reliable transportation
- Strong communication skills
