Compensation: $80,000 - $90,000 / week

Schedule: Exempt, min 40 Hours per week | 8:00am - 4:30pm | Monday - Friday

What You'll Do:

The Occupational Therapist is responsible for evaluating participants and developing individualized treatment plans that promote independence, improve functional mobility, and enhance quality of life. This role provides therapeutic interventions focused on activities of daily living, adaptive equipment, manual techniques, and environmental modifications in both clinic and community settings. Working collaboratively with an interdisciplinary care team, the Occupational Therapist delivers evidence-based care, monitors participant progress, and maintains accurate documentation to support positive outcomes and participant-centered goals at Huron Valley PACE.

Key Responsibilities
  • Conduct initial and ongoing participant evaluations, reassessments, and care planning to support optimal functional outcomes.
  • Develop, implement, and monitor individualized occupational therapy treatment plans and therapeutic interventions.
  • Provide therapy services focused on improving independence with activities of daily living (ADLs), mobility, safety, and overall function.
  • Assess the need for adaptive equipment and durable medical equipment (DME), and make recommendations to support participant independence.
  • Maintain accurate, timely documentation, including assessments, progress notes, treatment records, and discharge summaries within the electronic medical record (EMR).
  • Collaborate closely with an interdisciplinary care team to coordinate participant care and achieve evidence-based outcomes.
  • Participate in care plan meetings, case conferences, and discharge planning for participants transitioning between care settings.
  • Communicate participant progress, changes in condition, and treatment recommendations to team members, caregivers, and advocates.
  • Provide training and education to participants, family members, caregivers, and staff regarding therapy programs and adaptive strategies.
  • Supervise and provide guidance to program aides and other staff supporting occupational therapy services.
  • Contribute to quality improvement initiatives and organizational efforts to enhance participant care and service delivery.
  • Collaborate with the interdisciplinary team to identify and secure resources, equipment, and services that support participant goals.

Skills, Knowledge and Expertise
  • Graduate of an Occupational Therapy program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) or an equivalent accredited program.
  • Current Occupational Therapist (OT) license in the State of Michigan.
  • Current CPR certification or willing to obtain.
  • Minimum of one (1) year of experience working with a frail elderly population.
  • Valid driver's license, reliable transportation, and the ability to travel within the community as needed.
  • Ability to lift, transfer, and move objects weighing up to 35 pounds without assistance.
  • Strong assessment, treatment planning, documentation, and communication skills.


Preferred Qualifications

  • Two (2) or more years of Occupational Therapy experience.
  • Experience working in an outpatient, home health, geriatric, or PACE setting.
  • Experience collaborating within an interdisciplinary healthcare team.
  • Knowledge of adaptive equipment, durable medical equipment (DME), and home safety modifications.
  • Experience providing participant and caregiver education to support independence and quality of life.
