Job Overview

The Nurse Practitioner Street Medicine provides comprehensive care as part of AltaMed’s Street Medicine program, ensuring high-quality and professional care. The NP conducts patient visits for new and established patients experiencing homelessness and housing insecurity, coordinating their care plans with other members of the street medicine team as well as community service providers. The NP visits vary in settings, including street encampments, homeless shelters, and supportive housing sites. The NP will also make independent decisions regarding patients' healthcare needs. The Medical Director will be available for consultation, collaboration, and referrals as needed. This is a unique role for passionate nurse practitioners interested in working with unhoused populations.

Minimum Requirements

  • Graduation from an accredited Baccalaureate school of nursing.

  • Completed training in the diagnosis and management of common as well as some complex medical conditions.

  • Current licensure in the State of California.

  • Certification as an Adult/Family Medicine Nurse Practitioner.

  • Previous experience with computer-based systems is required.

  • Electronic Medical Records experience is preferred.

  • Previous experience in unhoused, high-risk populations preferred.

  • Bilingual in Spanish and English preferred.

  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation

$160,680.00 - $200,850.00 annually
