What You'll Do:As a
Primary Care Nurse Practitioner, you will:
- Provide comprehensive primary care to adult and geriatric patients.
- Perform patient evaluations, assessments, and follow-up visits.
- Diagnose and manage acute and chronic medical conditions.
- Develop and monitor individualized treatment plans.
- Manage common chronic conditions such as hypertension, diabetes, hyperlipidemia, COPD, CHF, and other medical conditions.
- Order and interpret appropriate laboratory and diagnostic studies.
- Review and manage medications.
- Provide preventative care and health screenings.
- Identify and address medical issues before they become emergencies.
- Educate patients and families about disease management, medications, lifestyle, and preventative health.
- Coordinate care with specialists and other healthcare professionals when appropriate.
- Help reduce avoidable emergency room visits and hospitalizations.
- Participate in quality-improvement and population-health initiatives.
- Communicate effectively with patients, families, caregivers, and interdisciplinary care teams.
- Serve as a trusted clinical resource for your patients.
- Practice with a high degree of autonomy while having support from the KMG team.
