Duties Include:
Developing treatment plans for acute and chronic diseases.
Educating and guiding patients on disease prevention and healthy lifestyle habits.
Demonstrates knowledge of the principles of growth and development over the life-span and the skills necessary to provide age-appropriate care to the patients served.
Performs diagnostic tests and screening evaluations.
Manages overall patient care regarding lifestyle and development issues.
Prescribes medications while using medication reference, as needed.
Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and family.
Revises plan of care as indicated by the patients' response to treatment.
Assists patients in managing chronic conditions, treating illnesses and helping patients make educated choices about their lifestyle, health and well-being.
Qualifications
Must have a current Indiana Nurse Practitioner or Physician Assistant license. (DEA, NPI, CSR)
Current BLS certification. ACLS preferred
Completion of required CME's.
Computer skills for documentation of patients charts in an EMR.
Ability to communicate in English; both verbally and in writing.
Additional languages beneficial, but not required.
Demonstrates the ability to be flexible, organized and function under stressful situations.
Must be able to multi-task as well as being a team player.
