Qualifications

• Full prescriptive authority is required.
• Graduate of an accredited nursing program.
• Current nursing license in the relevant state with a Master’s in nursing.
• Minimum of 1 year nursing experience needed.
• Current CPR certification required.
• Strong critical thinking and clinical expertise essential.
• Valid driver's license and auto insurance are mandatory.

Responsibilities

• Provide care in accordance with treatment and patient care plans for palliative care eligibility.
• Establish and adjust plans of care based on client assessments.
• Report any changes in patient conditions to necessary personnel.
• Deliver patient care education and counseling to clients and caregivers.
• Coordinate services with various disciplines and community resources as needed.
• Evaluate and modify care plans based on patient or environmental changes.
• Assist in developing educational programs for diverse age groups in palliative care.
