Job Summary:The Geisinger Skilled Nursing Program is seeking an experienced Nurse Practitioner or Physician Assistant at Oak Ridge Nursing Home in Taylor, PA.
Job Duties:This position is for a Geisinger provider working within a contracted skilled nursing facility. It requires an experienced advanced practitioner with prior clinical experience in skilled nursing, family medicine, internal medicine, or intensive care. The advanced practitioner (AP) provides direct (hands-on) care in skilled nursing facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.

Responsibilities:
- Review reports
- Communicate with SNF staff for patient needs
- Treat patients for acute needs, admissions, discharges, etc.
- Manage extremely complex patients, see every day or at least once a week
- Approximately 12 patients a day
- Length of time per patient and number of patients is per need
